The Adolescent Center for Treatment is pleased to announce the opening of a Case Management position. This position will provide direct client and family support services, conduct recovery/discharge planning, and follow up with clients after discharge. The Case Manager position will be responsible for working closely with the clinicians of ACT as well as with other outside providers and community support systems. The Adolescent Center for Treatment recognizes the importance of facilitating effective referrals to other providers both in the Johnson County area as well as throughout the state. This position will also be responsible for documenting ACT’s outcomes in these efforts to ensure that client’s receive services in their home communities once they are discharged from the program.. First review of applications will begin on September 16 2022. Other job duties include:
Bachelor’s degree in Human Services or related field and valid driver’s license with acceptable driving record required.
Experience may be substituted for degree. Education may be substituted for experience.
One (1) year of related work experience preferred.
Johnson County Government requires reference/background screening for all positions. Specified criteria may vary by Department/Agency.
Physical, Environmental, and Special Working Conditions:
All County employees may be called upon to assist other departments in a declared emergency situation.
